Best Medical Care has started receiving a very limited supply of Vaccines from the City of New York and following the strict eligibility guidelines as set-forth by the regularity authorities. We have started administering the vaccine on a pre-approved and pre-booked appointment schedule to our patients and the communities we serve based on the availability of doses.

What vaccine are we administering?

At this time, BMC is administering the Moderna vaccine to eligible groups. To learn more about the Moderna vaccine, click here.

The vaccine is administered in two doses separated roughly by 4 weeks.  Following your first dose BMC will contact you to schedule your 2nd dose appointment, or you can reach us at (718) 850-1673 after 3 weeks of first dose to get your 2nd appointment.




Preparation and documentary requirements for your vaccine visit

  • Complete the NYS Eligibility Form  to get vaccinated. You must show completion of this form when you arrive for your appointment. Either print the confirmation email you received with a GREEN CHECK MARK or simply forward it to our designated email address at the time someone reaches out to you for appointment
  • Read and complete the COVID-19 vaccine consent form. Bring the completed form with you on the day of your appointment.
  • Review the Moderna Vaccine EUA Fact Sheet for Patients.
  • Bring proof of eligibility to your appointment. This may include an employee ID card, a letter from an employer or affiliated organization, or a pay stub, depending on the specific priority status. If you are eligible because of your age, bring a government-issued ID that includes your date of birth (like a Driver’s License or passport) and proof of residence in New York, such as ID, landlord statement, or lease.
  • If you are a new patient to BMC you will also be required to register and sign HIPAA and other regularity paperwork, that will be provided to you at the time of your appointment.
  • The COVID-19 vaccine is free to you as it is covered by the state, however administration of this vaccine must be submitted directly to your health insurance provider or Medicaid and Medicare.  Please bring your health insurance card.
  • If you are enrolled in a Medicare or a Medicare Managed Care plan, please have your Federal Medicare ID number found on your RED, WHITE and BLUE Medicare Card available at your COVID-19 Vaccine/Booster visit. If you do not have your original Medicare Red, White and Blue card, our offices can look up this number for you with your Social Security Number.

In addition to the steps above, please enroll in v-safeThis is a CDC smartphone-based tool that uses text messaging and web surveys to provide personalized health check-ins after you receive a COVID-19 vaccination. Through v-safe, you can quickly tell the CDC if you have any side effects after getting the COVID-19 vaccine. Depending on your answers, someone from CDC may call to check on you. And v-safe will remind you to get your second COVID-19 vaccine dose at the right time.



What happens after you receive your vaccine shot?

Following your COVID-19 vaccine, you will be required to remain in the office for 15-30 minutes. Our nursing team will observe you and assess for any serious adverse reactions.  These are rare.  During the observation period, you will receive your Moderna vaccine card.  You will be required to bring this card on your 2nd dose visit.
